Volusia County Background Check Form.
Everyone willing to deploy during a Government activation must have a completed county background check.
To complete this check you must fill out this form but do not sign or date it. This form must be notarized by a notary public. You can have it notarized by places such as banks, law firms or for free at the county EOC (Emergency Operations Center) where you will be dropping off the form.
You must notify the EC of the date and person you left the document with. This way we can track the progress of your form. You can optionally provide a copy of the form to the EC to help in tracking or loss of the document.
